Accepted teams will be posted online by Saturday April 19th. No refunds will be issued once a team is accepted.
MandatoryCheck-in (at Pheasant Run Resort)
All teams must check-in on Friday May 23rd at the Pheasant Run Resort between 2:00pm and 10:00pm. There will be no Saturday check-in. Bring all necessary paperwork and a current player pass for each player.
Prior to check-in, mail two copies (except where noted) of the following forms to Campton United at the PO Box listed above.
Forms must be received by Friday, May 9th.
State Association Approved Team Roster, including any Drop/Add Forms.
State Association Tournament Roster, completed and signed by the team's coach.
Guest Player Forms (if applicable), completed and signed by coaches of BOTH teams.
One copy of the IYSA Medical Release Form for each player. No out of state forms will be accepted. All information must be unaltered, with nothing crossed-off or scratched-out.
State Association approved Application To Travel for for all out of state teams. This applies to teams outside the state of Illinois who are traveling to the Midwest Cup Tournament. Out of state team need to submit the travel permit from their home state's governing body.
Mailing in the team registration forms in advance helps expedite Firday's on site check-in process. In addition to mailing the team registrations forms, a representative from each team must stop by the check-in table at the Pheasant Run Resort to verify player passes and have a tournament ID attached to them. The tournament ID will show the referee that your team has been properly registered and checked-in for the tournament.
